Requirements:

Board Certified in Neonatal-Perinatal Medicine - [REQUIRED]
VA license (or willing to obtain) - [REQUIRED]
Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
